Wholesale Application

You must fax us a copy of your sales tax permit to be approved.  

Your main business phone number will be your account number. Entering this number in the appropriate coupon field will allow the wholesale discount to be applied for all qualifying orders.

All items purchased must total $100 or more at regular pricing to be eligible for a wholesale discount. Orders that do not meet this requirement will not be discounted.

Freight charges will be the actual freight plus $1.00 per order. All other sections of our policies pertain to all wholesale orders. This includes orders under $40 at regular pricing will be subject to the Small Order Handling Charge.

You must fax us a copy of your sales tax permit to be approved. We will email your password upon receipt of this form and a copy of your sales tax permit. Our fax # is 918-622-8063.
